Registered Nurse - Patient Care Coordinator

Assessment will include complex medical, emotional, social, spiritual and financial situations.
Coordinate referrals to both community-based services and Providence Support Services.
Serve as the “hub†of multiple lines of communication between the patient, referring physician and support services providers.
Encompass the roles of liaison, communicator, educator and supporter.
Work a 1.0 FTE, Day Shift
Required qualifications for this position include:
Graduate of an accredited school of nursing
Current Registered Nurse (RN) licensure in Washington State
Minimum of 5 years applicable clinical experience
Preferred qualifications for this position include:
Bachelors of Science in Nursing (BSN)
Certification in Case Management or the ability to obtain within two years of employment
