The UWMC Pharmacy has an outstanding opportunity for a full-time PHARMACIST 3 to join our General Surgery Pharmacies team! The Pharmacist 3 will be responsible for providing clinical services for the Surgical ICU and the General Surgery services in ! collaboration with other clinical pharmacists. The general sur! gery service covers a diverse surgical population including patients having abdominal, colorectal or head and neck surgical procedures. This service also coordinates care for patients undergoing gastric bypass surgery. The clinical pharmacist is responsible for the medication management needs of the Surgical ICU service. The clinical pharmacist is an essential member of the health-care team whose primary role is to ensure that their assigned patients are receiving safe, appropriate and cost-effective medication therapies.
Additional responsibilities include:
Medication reconciliation, ensuring medication dosing and monitoring, writing TPN orders under a prescriptive authority protocol, ensuring antimicrobial stewardship by directing empiric therapy and streamlining based on culture and sensitivity data.
In addition to direct patient care, the pharmacist will provide drug therapy consults, participate in treatment protocol development, provide st! aff/patient and family education, and precept pharmacy students and residents.

Requirements:
Pharmacy degree (eligibility for State of Washington Pharmacist License) AND two years of experience in pharmacy practice.
Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the required experience. Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration.
