This job is responsible for improving patient care and achieving quality! , cost-effective and patient-focused clinical outcomes by serving as an advanced practice clinician, educator, consultant, leader and researcher for a specific patient population (e.g. cardiovascular/surgical critical care, med/surg, etc) in accordance with Franciscan Health System (FHS) standards/guidelines, applicable statutory requirements and accepted clinical practice for the clinical nurse specialist profession. An incumbent serves as a nursing practice expert to treatment teams, with the expertise to deal with a large number of variables relative to all aspects of nursing process to include assessment, plan of care, implementation of interventions with emphasis on those necessary to address complex treatment issues and determine specific course of action to meet patient's needs and to attain and maintain optimum quality of nursing care.
Work also includes creating/maintaining standards and systems for care delivery for the defined patient population. These system! s of care delivery promote compliance with evidence-based prac! tice and CMS quality of care indicators, and are designed to support the management of productivity targets within the context of constant change in patient volumes, patient acuity, and available resources. An incumbent promotes compliance with evidence-based best practices and CMS quality of care indicators, and facilitates nursing staff development through involvement in clinical practice, education, consultation and nursing research activities.
Work requires strong clinical nursing expertise and an understanding of educational concepts, patient care standards/practices and applicable regulatory requirements specific to the assigned specialty area. An incumbent exercises a high degree of discretion/independent judgment and applies advanced skills obtained through extensive clinical preparation and graduate-level academic study. Situations encountered entail the application of sound critical thinking skills in making decisions regarding patient care (within the scope o! f practice), the integration of available standards, resources and data, and in the efficient utilization of staff/resources. Knowledge of adult education/quality improvement concepts, methodologies and tools, and of the importance of data to performance improvement processes is also required. Work is performed in collaboration with hospital management, nursing leadership, physicians and other departments in the advancement of staff competency, clinical quality and achievement of core measures quality indicators.
Esssential Duties:
Provides expert nursing practice in a defined area of clinical responsibility within scope of practice:
Leads and coordinates the ongoing delivery of patient care services provided by members of assigned patient care team(s); considers patient acuity and the competencies of care team members in delegating and assigning work and in adjusting assignments as necessary;
Interprets and facilitates the application of standar! ds of nursing practice to the patient care process; implements appropri! ate interventions and evaluates outcomes; assesses, interprets and intervenes when patient condition or progress varies from pathway/standard;
Assesses and monitors team performance on a continuous basis to ensure the quality of care provided is consistent with FHS standards and regulatory requirements.
Consults with physicians, professional nursing staff and ancillary members of the patient care team in determining nursing care/treatment plans/case management and to address clinical practice issues and changes:
Evaluates general management of patients and patient's response to nursing interventions and multi-disciplinary treatment plans;
Serves as a leader/consultant/mentor/change agent in advancing the practice of nursing among other nurses, using innovative evidence-based interventions, best-practice guidelines and modification of professional standards and organizational policies that direct the care of nursing personnel and other healthcare providers to! improve outcomes.
If assigned to Perioperative Services:
Partners with designated medical staff leaders as needed to accomplish goals/initiatives;
Establishes written guidelines (e.g. standards of care, policies, procedures and protocols) consistent with applicable regulatory and accreditation requirements and professional standards relating to perioperative services.
Designs, implements and evaluates new and innovative programs of care to achieve desired evidence-based, quality, cost-effective and nursing-sensitive outcomes:
Applies expert knowledge of different illnesses, diagnoses and treatments in comprehensive, holistic assessments of patients and development of new standards of practice which assure the delivery of care in a safe and cost-effective manner;
Maintains current knowledge of departmental policies/standards/procedures, JCAHO standards, new clinical procedures/processes, patient safety goals, core measure quality indicato! rs and other issues/requirements impacting matters within assigned scop! e of responsibility; ensures that these factors are considered and integrated into clinical, performance improvement and education activities.
Develops sustainable structured training programs and coordinates/delivers in-service education/training specific to the cardiovascular, critical care or perioperative departments:
Provides consultative services to clinical staff to maintain and improve knowledge and competency in a wide variety of practice skills; instructs/orients nursing and paraprofessional staff in the techniques for providing patient care and advances in the nursing specialty;
Interprets and explains requirements, standards and procedures as required; plans and carries out staff development programs through direct counseling/guidance, structured learning experiences and patient-centered conferences; ensures that educational activities provide staff with skills and competency needed to meet all applicable regulatory requirements;
Assists in formu! lating and implementing educational and administrative policies for nursing; assesses education needs of nursing staff and/or works collaboratively with unit-based educators to plan nursing education programs;
Promotes and participates in local/national professional organizations and maintains professional growth and development; contributes to the community of nursing and the advancement of nursing knowledge within the profession.
Identifies clinical nursing problems/issues for research study:
Initiates and participates in studies for improvement of patient care, education programs and organizational effectiveness; meets with members of other clinical disciplines and community healthcare providers relative to matters involving nursing practice; assists staff in applying research findings in actual practice;
Evaluates current published literature and interprets information in order to make decisions based on proven research; conducts research involving hospi! tal programs and system effectiveness.
Provides continuing assistan! ce to management with respect to the development and/or improvement of existing clinical/treatment protocols/practices through the ongoing observation, analysis and evaluation of current activities:
Monitors and evaluates results achieved in relation to objectives; participates in the development, evaluation, implementation and ongoing maintenance of policies, standards, procedures and automated tools that support FHS goals and department-specific objectives/initiatives; serves as department representative on cross-functional and interdisciplinary teams as assigned;
Keeps abreast of and new/changing regulatory requirements impacting matters within designated scope of responsibility; prepares training materials and delivers staff development and in-service education relating to regulatory requirements and compliance issues; serves as a resource to physicians and hospital staff regarding the full spectrum of compliance issues.
Attends and actively participates in ! staff meetings; presents and discusses clinical concerns and issues; demonstrates awareness of, and consistently follows, established departmental policies/procedures and quality improvement, safety, environmental and infection control standards; identifies and reports hazards and/or environmental concerns in a timely manner.

Patient Population Served: Demonstrates knowledge and proper skills associated with the department's defined specific populations served.
Performs related duties as required.
Education/Work Experience Requirements:
Master's degree in Nursing from an accredited college or university and a minimum of three years of progressively responsib! le RN work experience in the applicable specialty area. Successful comp! letion of a clinical nurse specialist program is strongly preferred.
If assigned to Critical Care, RN work experience must be in cardiovascular critical care (e.g. open heart recovery). If assigned to Perioperative Services, RN work experience must be in OR/PACU, and nursing informatics experience is preferred.
License/Certification:
- Current licensure as a Registered Nurse issued by the Washington State Board of Nursing.
- Current CCRN (Critical Care Registered Nurse), CSC (Cardiac Surgery) Certification, and/or CNS (Certified Nurse Specialist) Certification is preferred.
- If assigned to Perioperative Services, CNOR (Certified Nurse, Operating Room) is required; ACLS preferred.
